Voucher Authorization Form

Complete and sign the Voucher Authorization Form.
You must attach a copy of a valid state-issued ID to the form.
Give the name and daytime phone number of the person who will be bringing items to the South Transfer Station.
Email the form to our office.
Call us to confirm that we received and approved the authorization.
You can then schedule a time to use the voucher.
