Prepaid data request account form
You can create a prepaid account with the City to pay for data request charges.
What to do
- Download and complete the form.
- Send the completed form with a check or money order to the Minneapolis Police Department Records Unit.
- Submit your data request
Please note:
- Accounts with a zero balance become inactive. We cannot complete your data request until we receive payment.
- We'll tell you when your account balance is below $50.
