Using the Reporting Person Form
This Form collects the contact information of the person making the Police report.
There are several pieces of information required from the Reporting Person. Data is collected in accordance with state and federal guidelines.
Name: Enter your first name and last name (middle name or initial is optional).
Date of Birth: Enter your birth date in the mm/dd/yyyy format. If you are under 18 years of age, your parent or guardian information will be requested.
Gender and Race: Select from the options provided on the drop down lists.
State Identification Number: Enter your driver’s license number or other government issued Identification number (state ID, International Driver’s License, passport, etc).
State: Defaults to MN. If license/ID is from a different state, select it from the dropdown menu. If your license/ID was issued outside of the United States, leave the state set to MN. You can add a note about country or agency that issued the ID in one of the text boxes found on the upcoming "Incident Details" form.
Phone number: You must enter at least one complete phone number (area code, number, and type of number – home, fax, cell, etc.). If you do not have a phone number, type zeros in the phone fields and select "Other Phone" from the Phone Type list.
Email address: This is not a required field but it must be filled in with a valid email address if you would like the report emailed to you when you’re done.
** New: The official Minneapolis Police Report will automatically be sent to the email address provided here. You can print a copy or save this file.
Address: Use the -Select Address- feature to begin entering the Reporting Person’s address. Click the down arrow to view choices.
If an actual street address was used to identify the Incident Location (not an Intersection or Landmark), you will see that full address listed here. Click on the address to select and record it as the Reporting Persons address or click/select "Enter another address…" and enter a different address on the following form.
If an Intersection or Landmark was used as the Incident Location, you will not see a corresponding street address on this list and will have to enter an address on the next form. Click/select "Enter another address…".
When you are finished entering all data on this form, press "Next>>". If you selected "Enter another address" in the preceding step, you will now be at the Enter Reporting Person Address
Entering the Reporting Person’s Address:
Enter the address information of the person making the report.

Check the data entered and click "Next>>" when finished.
