Using the Date / Time Form

About this form

Date and time values must be entered in the specified format.

Datem/d/yyyy, for example: 3/21/2006
Timeh:mm, for example 12:00 or 1:00 and select AM or PM

The first Date field cannot be blank. Type in or click on the calendar icon to enter the date on which the incident occurred or began. If you have no idea of when it happened, enter a ‘best guess’ date. If the exact times are unknown, you may approximate or leave the Time field blank.

When the incident occurred on a specific date and time, enter that information in the top "Incident Date and Time" fields and leave the "To" fields blank.

Enter a range of dates/times to indicate the incident occurred within a specific timeframe. For example, if you were reporting a stolen bicycle, you would enter the date and time you last saw the bike as the starting date/time. The ending ("To") date and time would be when you discovered the bike missing.

Dates cannot be "in the future" and not more than one year "in the past". The system will only allow you to enter a date on or before today’s date but no further back than one year from today’s date.

Times must be entered in a valid sequence. The system will not let you set a start time that is "after" an end time on a given date.
