*Plan payments for diagnostic and preventive services are included in the benefit maximums noted below.
If you use the services of a Delta PPO dentist, the plan will pay up to $1,500 per calendar year for each covered person. If you use the services of a Delta Premier or out-of-network dentist, the plan will pay up to $1,000 per calendar year for each covered person.
An annual deductible of $50 per year for each covered person applies to basic, major, and prosthetic services. The annual deductible will not exceed $150 per family. The annual deductible does not apply to diagnostic, preventive or orthodontic services. The plan document contains detailed information on the services that are eligible for coverage and the procedures you must follow to obtain benefits.
Enrolling for Dental Benefits
Most full-time employees and their eligible dependents may enroll in benefits the first of the month following 30 days of employment. Certain part-time employees are eligible to participate in the dental plan. Eligibility and contribution requirements for part-time employees are based on the governing policy or collective bargaining agreement.
Changing Your Dental Plan Elections
You can change your dental plan election, including adding and deleting dependents, during the year with a qualified event. To change your election, you must complete a Health Plan Enrollment/Change Form and return it to the City’s Benefits Office.
Orthodontics is subject to a separate lifetime maximum of $1,000.00 per Covered Person and limited to those Orthodontic treatment plans commenced on or after the Eligible Dependent Child’s eighth (8th) birthday and through the Eligible Dependent Child's eighteenth (18th) birthday.
