Using the Motor Vehicle Forms
About the forms used to collect information on the vehicle(s) involved
Vehicle Information - data entry form
This form collects required information about a motor vehicle involved in the incident. If more than one vehicle is involved, enter each on its own form.
"Make" and "Style" of the vehicle
Make is the manufacturer of the vehicle. If the Make is not listed, select "Other Domestic" or "Other Foreign" and then enter the Make in the text field labeled "Other:".
If you don’t know the Make, select "Unknown" from the list.
Style is the body type of the vehicle. If your choice is not on the list, select "Other Vehicle" and enter the body style in the text field labeled "Other:".
Was the vehicle damaged? - Yes or No
YES - When you select "Yes, the ‘Damage Description’ and ‘Estimated Cost of Damage’ fields activate. Use them to describe what was damaged, type of damage, location of damage, etc. Be thorough in your descriptions.
If you do not yet know the dollar amount of the damage, enter an estimated value in the ‘Estimated Cost of Damage’ field.
If you are reporting ‘Damage to Property (Motor Vehicle)’, enter the vehicle damage on this form. You will use a different screen (the Property Information form) to list any other property that was stolen and/or otherwise damaged.
NO - If the vehicle was not damaged, select "No". The ‘Damage Description’ and ‘Estimated Cost of Damage’ fields are inactive and are grayed out, as shown below.
Who is the owner of this vehicle? – Pick a name
Owner’s name is not listed. You can add the owner’s name and information (see above). Select "Another Person or Business" and press "Next>>". You’ll move to the "Vehicle Owner Information" form where you can enter the owner information.
Owner’s name is listed. Select the owner’s name and press "Next>>". System moves to the Vehicle Information summary screen, shown below.
Vehicle Information – review list form
This summary page lists all vehicles entered so far in the report. From here you can add more vehicles, remove vehicles, and/or edit existing vehicle and owner information. Screen shot above shows one vehicle listed.
To edit vehicle or owner information, press the [Edit] button for the entry you want to change.
If you would like to delete a vehicle entirely, press the [Delete] button next to the vehicle you want to remove from the report.
Was there an additional vehicle involved in this incident? Yes or No
YES - If there are more vehicles to enter, select "Yes, Add Vehicle" and press "Next>>" (see below). A new Vehicle Information screen opens for you to enter the information on the next vehicle. Repeat as needed for additional vehicles.
NO - If there are no more vehicles to enter, select "No" and press "Next>>".
