Using the Legal Disclaimer Form

You are required to accept the legal disclaimer before you begin filing your On-Line Police Report.

This text reminds you that you are completing an official police report. Any attempt to use this system to commit fraud or any other illegal acts is punishable by law.

When you click "I Accept", the City of Minneapolis assumes that you are reporting the crime you describe honestly and as accurately as possible.

If, after completing the form, you feel you may have made a mistake entering details about the incident, or discover that the crime you reported did not occur as you described it, contact the Minneapolis 311 Call Center at 3-1-1 or (612) 673-3000, weekdays, 7:00 a.m.-7:00 p.m. Tell the Agent you need to amend your police report. Your call will be transferred to the appropriate Minneapolis Police Unit. Please have the case number of your report available when you call.
