Last chance to earn substantial health insurance savings

There are only 10 days left to finish Medica’s My Health Rewards program and qualify for the lower health insurance premium rate in 2014. If you’re close to completing the program, be sure to finish by the Aug. 31 deadline.

Employees who don’t complete the program will be paying about $400 more for single health insurance coverage or $1,200 more for family coverage in 2014 than the folks who finish the program.

Close doesn’t count

Don’t miss substantial savings because you’re short a few points. To qualify for the lower premium rate, you must earn 300 points by Aug. 31, 2013. If you intend to finish the program, take time to work on it now because you will not be able to do it all at the last minute.

How to check your points

How many have you earned? Do you still need to earn additional points? Have you checked Medica’s website to verify you’ve been credited for the wellness activities you completed?

Here’s how to check your points:

  1. Log onto mymedica.com.
  2. Click on the Health & Wellness tab.
  3. Click on Personal Health Scorecard.

Points you have earned and points you are eligible to earn will be listed on your scorecard.

How to earn points

To qualify for the lower premium rate in 2014, you must earn 300 points by Aug. 31, 2013 by completing any combination of the health and wellness activities listed below. Please note: the deadline has passed for some activities.

Personal Health Profile (100 points) – Complete a short health questionnaire by logging onto mymedica.com and clicking on the Health & Wellness tab. In addition to earning 100 points, you will receive a $20 gift card.

Health Topics and Goals (100 points) – Read eight new health topics and set health goals. There are hundreds of topics to read about and they can be found on mymedica.com (click on the Health & Wellness tab). Completing all eight topics and setting eight health goals will also earn you a $40 gift card.

Biometric Screening (100 points) – Although all worksite screening sessions have passed, you can still complete the screening by visiting your doctor’s office to obtain lab results for blood glucose, total cholesterol, HDL, LDL, triglycerides, blood pressure and body mass index. Be sure to submit Medica’s Health Action Notification form along with the lab result sheet from your doctor’s office. Bring the Health Action Notification form with you to your doctor’s office to make sure you have completed the correct lab tests.  Submit the Health Action Notification form and lab results by mail or fax to the contact information on the form.

Fit Choices by Medica (100 points) – If you were enrolled in Fit Choices and worked out at a participating fitness center eight times per month for at least four months between Jan. 1, 2013 and July 31, 2013, you qualify for 100 points. Check your scorecard to make sure you have been credited for your workout sessions.

Medica’s health assessment is administered by an independent contractor, and your individual answers are not shared with either Medica or the City of Minneapolis.
