Completing a health profile is an easy way to begin qualifying for reduced health insurance premium

Completing Medica’s Personal Health Profile is a great way to take charge of your health, earn a $20 gift card and earn 100 of the 300 points needed to qualify for the reduced health insurance premium in 2014.

To complete your Personal Health Profile

Answering the questions to create your Personal Health Profile takes only 10 to 15 minutes. Here’s how to access the profile:

(If you’ve never signed in to mymedica or you have a new Medica group number, you will need to register as a new user. (View new user registration instructions.)

After answering the assessment’s wellness-related questions, you will be able to view the results of your Personal Health Profile. The website will also suggest health topics you may wish to read. (Note: You are not required to read any of the topics to receive points for completing the health profile.)

About the Personal Health Profile

The health assessment is administered by an independent contractor, and your individual answers are not shared with either Medica or the City of Minneapolis. Only aggregate numbers are provided to Medica so it can map health trends and offer additional services to members who might benefit from those services.

Obtaining your $20 gift card

One to five days after you have completed your Personal Health Profile, you will receive an email linking you to a site where you can select a $20 gift card from one of the participating shops and restaurants.

Keeping track of your points

Employees who earn 300 points for completing any of the program’s health actions by Aug. 31, 2013 will qualify for the lower wellness rate on their 2014 health coverage.

To see how many points you have earned

Points you have earned and points you are eligible to earn will be listed on your scorecard. Please note: the scorecard does not immediately update after taking the assessment. You may have to wait several days to see that your points are recorded.
