1. Communities Transformation Grant Application:
Authorize appplication for a 2-year Communities Transformation Grant, Small Communities Program grant from the U.S. Department of Health and Human Services, for an amount not to exceed $3,825,780, to reduce chronic disease rates among Minneapolis residents. (Refer to W&M/Budget)

2. University of Minnesota School of Nursing Affiliation:
Authorize five-year Institutional and Program Affiliation Agreement with the University of Minnesota School of Nursing to provide clinical experiences for nursing students.

3. Civil Rights Commissioner Appointments:
Set public hearing for Wednesday, July 25, 2012, at 1:30 p.m. on an ordinance amending Title 7, Chapter 141 of the Minneapolis Code of Ordinances relating to Civil Rights: Administration and Enforcement, providing for Civil Rights Commissioner appointments to end on December 31 instead of August 31 and providing that such appointments shall be made under the open appointments process.
