1.   Commissioner of Health/Director of Department of Health:
Consider the charter department head reappointment by the Executive Committee of Gretchen Musicant to the appointed position of Commissioner of Health/Director of Department of Health for a two-year term beginning January 2, 2014.

2.   Public Health Advisory Committee Appointments:
Approve the following appointment, reappointments, term corrections and updates to the Minneapolis Public Health Advisory Committee: Dr. Siyad Abdullahi (Ward 3), Saeng Kue (Ward 4), Karen Soderberg (Ward 7), Dr. Rebecca Thoman (Ward 13), Linda Brandt (Hennepin County Human Services and Public Health), Julie Ring (Ward 1), Birdie Cunningham (Ward 11), Autumn Chmielewski (Ward 12), Jennifer Pelletier (U of M School of Public Health Rep), Dan Brady, (Member At Large), Tara Jenson (Ward 5), Dr. Happy Reynolds-Cook (Ward 6), Abdullahi Sheikh (Ward 8), and Joseph Colianni (Member At Large).

3.   Affiliation Agreement for Medical Residents in School Based Clinics:
Authorize execution of a five year affiliation agreement with the University of Minnesota through its Family Medicine Residency Program to provide a medical rotation site through the School Based Clinic Program.

4.   Centers for Disease Control and Prevention Grant Application.
Authorize application for a 3-year grant from the Centers for Disease Control and Prevention (CDC) for an amount not to exceed $750,000 beginning September 1, 2014. The purpose of the grant is to develop multi-sector partnerships to promote sexual health, advance community wellness, influence sexual health behavior and practices, and reduce STD disparities.
