Complaint Investigations Division - Civil Rights Complaint Form

Civil Rights Complaint

If you believe that you have been discriminated against and the alleged incident took place in the City of Minneapolis within one year prior to filing a complaint, please fill out the below information:

Reporting the exact address is very important. Please fill out as much information as you know.

For example: "I was terminated on May 3, 2011 by my employer because of my gender," or "I was refused service at a coffee shop because of my race," Not: "I was fired from work." or "I was denied service."

Once the Department receives your completed complaint request form, an investigator will contact you to schedule an intake interview. Important: Completing the complaint request form or the intake questionnaire below does NOT formalize a charge of discrimination with the Department.
