Civil Rights Complaint

If you believe that you have been discriminated against and the alleged incident took place in the City of Minneapolis within one year prior to filing a complaint, please fill out the below information:

Reporting the exact address is very important. Please fill out as much information as you know.

For example: "I was terminated on May 3, 2011 by my employer because of my gender," or "I was refused service at a coffee shop because of my race," Not: "I was fired from work." or "I was denied service."

Once the Department recieves your completed complaint request form, an investigator will contact you to schedule an intake interview. Important: Completing the complaint request form or the intake questionaire below does NOT formalize a charge of discrimination with the Department.
